CareConnect is a leading provider of home health care services. We are seeking a qualified Registered Nurse to join our team in Franklin County, MA.
Job Overview:
- We're looking for compassionate and skilled nurses to provide exceptional care to patients in their homes.
- The ideal candidate will be a registered nurse with a current license in the state of MA and at least one year of experience in acute care, home healthcare, community health, or outpatient facility settings.
Key Responsibilities:
- Perform initial and ongoing patient assessments based on patient needs and physician orders.
- Develop and update a written plan of care, ensuring it is re-evaluated as necessary.
- Provide skilled nursing care within the nursing scope of practice.
- Document care and services in compliance with federal, state, and Agency regulations.
- Support patient/family cultural practices, provided they don't interfere with medical treatment.
- Promote patient, family, and caregiver education tailored to cultural, ethnic, and personal needs or preferences.
Qualifications:
- A graduate of an accredited school of nursing.
- A valid state driver's license and reliable automobile.
- A strong understanding of HIPAA regulations and commitment to maintaining patient confidentiality.
- Ability to communicate effectively with patients and the healthcare team.
- Fluency in English, both spoken and written.
Compensation and Benefits:
- $45 per visit.
