About the Role: We are currently seeking a skilled Licensed Practical Nurse (LPN) to join our team. As an LPN with CareConnect, you will play a vital role in providing direct patient care and implementing individualized plans of care. Your expertise and compassion will enable our clients to receive the highest level of service possible, enhancing their overall quality of life.
Duties and Responsibilities:
- Provide direct patient care in accordance with state nurse practice acts
- Implement individualized plans of care initiated by registered nurses
- Ensure accurate and timely documentation
- Assess and educate patients and their families/caregivers on self-care techniques and relevant information related to diagnosis and care plan
- Coordinate home health services
- Use and prepare medical equipment and supplies effectively
Qualifications: To be successful in this role, you must have a current, active license as an LPN in Massachusetts. A minimum of three years of nursing experience is required, with a strong preference for experience in community/home health or medical/surgical settings. Additionally, you must hold up-to-date CPR/BLS certification and provide proof of eligibility to work in the U.S.
