We're seeking a Patient Advocate to join our team at CareConnect. As a Patient Advocate, you'll have the opportunity to work closely with patients and their families to ensure that their needs are met and their rights are protected. This rewarding role requires a caring and compassionate individual who is able to provide a high level of advocacy and support to our patients.
Responsibilities will include assisting patients with daily living activities, providing emotional support and companionship, and maintaining a clean and safe environment for patients. You'll also have the opportunity to administer medications as directed by healthcare professionals.
Requirements:
- Certified Home Health Aide (HHA) certification in the state of NY
- Previous experience working with individuals with developmental disabilities, seniors, or patients requiring home care
- Reliable transportation
- Able to work a minimum of 35 hours/week and every other weekend
