About the Role
We're looking for skilled physical therapists to join our team in Westchester, Rockland, and Bronx Counties. As a key member of our team, you'll provide high-quality care to patients in their homes, working closely with other healthcare professionals to achieve optimal outcomes.
Responsibilities
Your primary responsibilities will include:
- Assessing patients' functional limitations and developing personalized therapy plans.
- Providing direct care services to treat medical and surgical conditions using accepted physical therapy modalities.
- Instructing and supervising patients, families, home health aides, and others on home exercise programs and patient care aspects.
- Continuously assessing patients' conditions and adjusting therapy plans as needed.
- Reporting significant clinical information to physicians, clinical managers, and other team members.
Requirements
To excel in this role, you'll need:
- A NY Professional License and current registration as a Physical Therapist.
- Minimum 1 year of full-time experience providing physical therapy services.
- Ability to assess and respond to patient and family needs in a home care setting.
- Home care experience is preferred but not required.
- Ability to use technology (such as tablets) for patient documentation and care.
- Experience with Oasis assessments is a plus.
- A valid driver's license and access to a reliable automobile for travel in the field.
