**Key Responsibilities:**
- Assess patients' needs and develop personalized care plans in collaboration with the registered nurse.
- Provide direct patient care, including administering medications, wound care, and vital signs monitoring.
- Communicate effectively with patients, families, and caregivers to ensure seamless care transitions and coordination.
- Document patient information accurately and timely, adhering to regulatory requirements and maintaining confidentiality.
- Collaborate with interdisciplinary teams to coordinate home health services, ensuring comprehensive care delivery.
**Requirements:**
- Current licensure as a licensed practical nurse in the state of Massachusetts.
- Minimum three years of nursing experience, preferably in community/home health or medical/surgical settings.
- Up-to-date CPR/BLS certification.
- Valid state driver's license and reliable automobile.
- Authorization to work in the U.S. and proof of eligibility, if necessary.
- Fluency in English, both spoken and written, to communicate effectively with patients and the healthcare team.
- Strong understanding of HIPAA regulations and commitment to maintaining patient confidentiality.
- Current health certificate/physical examination and TB testing results.
- Age 18 or older.
