About the Role
In this role, you will provide direct patient care in the comfort of our clients' own homes, working closely with our registered nurses and other healthcare team members to deliver high-quality patient outcomes.
Your Key Responsibilities
- Implement the plan of care initiated by the registered nurse
- Ensure accurate and timely documentation that aligns with the patient's plan of care
- Assess and educate patients and their families/caregivers on self-care techniques and relevant information related to the patient's diagnosis and care plan
Requirements
- Must be a graduate of an accredited practical nurse or vocational nursing program
- Must hold a current, active license as an LPN in the state of Massachusetts (MA)
- Must have an up-to-date CPR/BLS certification
- Must have a minimum of three years of nursing experience
