Medical Concierge

Carecore at Mentor is a Long Term Care facility seeking a Medical Concierge to support our residents and staff by ensuring smooth transportation scheduling, maintaining essential supplies, and assisting with daily operations. We are located in Mentor, Ohio.

Responsibilities:

Central Supply:

• Order, organize, and stock supplies on designated days
• Maintain the Central Supply room and stock nutrition rooms
• Track and fulfill supply requests from nursing staff
  
  
  

Transportation Coordination:

• Schedule and document resident appointments
• Communicate with nursing staff and drivers to ensure seamless transport
• Utilize PCC for scheduling and documentation
• Coordinate transportation to maximize efficiency, especially around dialysis schedules
• Arrange community transport for eligible residents and maintain updated records
  
  
  

Resident & Facility Support:

• Maintain daily shower sheet binders and ensure completion
• Assist with showers if staffing shortages occur
• Ensure water is passed on all three units each morning and evening
• Support dining areas by ensuring adequate staffing and assisting with meals as needed
• Attend daily morning meetings to review transportation and supply needs
• Step in to assist on the floor when necessary
  
  
  

Qualifications:

• Experience in healthcare, long-term care, or a related field preferred
• Strong organizational and multitasking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and as part of a team
• Proficiency with scheduling and documentation systems (PCC experience a plus)
