Job Summary :
The Case Manager provides comprehensive case management services to individuals assessed as needing addiction treatment. This role involves creating and implementing individualized service plans, facilitating group support, and coordinating referrals to promote client recovery and well-being in alignment with FLACRA’s mission, vision, and values.
Required Qualifications :
- Associate’s Degree in Health or Human Services.
- Bachelor’s Degree preferred.
- Valid NYS Driver’s License.
- Pursuit of Qualified Health Professional (QHP) status is encouraged.
- Experience in a Substance Use Disorder setting preferred.
