Pay Range : $25-$30 / hr. on w2
Job description :
The Quality Management (QM) Health Navigator position focuses on empowering and uniting New Yorkers through health care services, with an emphasis on ensuring that members receive essential health screenings, services, and medications. The role requires providing support to clients, helping them navigate the healthcare system, coordinating necessary appointments, and addressing barriers to care. The Health Navigator will also collect data related to healthcare barriers and social needs, helping shape the organization’s strategy for care access and health equity.
Key Responsibilities :
- Outreach & Coordination : Contact members to ensure they complete important screenings and follow-up on healthcare needs, such as scheduling appointments, refilling maintenance medications, and arranging transportation.
- Health Education : Educate members on the importance of regular screenings and healthcare services, using motivational techniques to encourage engagement in care.
- Data Collection : Gather information on barriers to healthcare and social needs, which will inform future care strategies and network improvements.
- Collaboration : Work closely with Care Management and other departments to ensure continuity of care and enhance member satisfaction.
- Documentation : Maintain accurate records of member interactions and data related to health care gaps, ensuring compliance with regulations.
