Job Summary
Careful Care is a community-focused housing stabilization provider serving individuals with disabling conditions in the Twin Cities. We're looking for a dedicated Program Manager to lead and support our growing team of Housing Transitioning and Sustaining Specialists.
Duties
- Lead and manage program initiatives related to property management, ensuring alignment with organizational goals.
- Oversee daily operations of the Housing Stabilization program
- Supervise, train, and support Housing Specialists (transitioning and sustaining)
- Monitor caseloads and ensure proper distribution across the team
- Track team performance and weekly KPIs using Notion and other tools
- Ensure compliance with DHS requirements and billing documentation
- Coordinate smooth client transitions from housing search to long-term stabilization
- Work closely with virtual assistants and onboarding partners
- Support company leadership in system development and process improvement
Qualifications
- Bachelor’s degree in Social Work, Business Administration, or a related field; Master’s degree preferred.
- 2 years in housing stabilization, case management, or social services
- 1 year supervisory or team lead experience (required)
- Familiarity with Minnesota Medical Assistance (MA) and DHS programs
- Strong communication, organization, and time management skills
- Tech comfortable (Notion, Google Workspace, EHR systems)
- Passion for helping vulnerable populations achieve stable housing

Pay: $25.00 - $31.00 per hour
