- Developing and implementing individualized care plans in collaboration with the interdisciplinary team
- Providing routine medical care, including medication management, wound care, and symptom management
- Maintaining accurate and up-to-date records of participants' health status and care plans
- Collaborating with family members and caregivers to ensure consistent communication and support
- Participating in ongoing education and training to stay current on best practices and evidence-based care guidelines
