Schedule :
9 : 00 AM - 5 : 00 PM
70% in-office; travel required to patient homes in MA, Brooklyn, Bronx, & Queens
Average caseload : 15-20 consults per day
About the Role : We are seeking an experienced Wound Care RN to join our team. This role requires expertise in wound assessment, treatment planning, and patient education. The nurse will collaborate with interdisciplinary teams and may conduct home visits.
Responsibilities :
Review potential wound care patients for Home Care Program eligibility.
Conduct comprehensive assessments for deep wounds, ostomies, burns, pressure injuries, and diabetic foot wounds.
Develop individualized wound / ostomy treatment plans in collaboration with caregivers and physicians.
Ensure evidence-based wound care practices are implemented and maintained.
Provide education and counseling on wound care, infection prevention, and risk factors.
Maintain professional relationships with medical staff, case managers, and external agencies.
Document patient progress and ensure compliance with clinical care plans.
Evaluate and train field nurses on wound / ostomy care best practices.
Coordinate communication and documentation between patients, medical professionals, and referral sources.
Participate in interdisciplinary meetings, quality improvement initiatives, and regulatory compliance.
Conduct case load reviews, chart audits, and ensure compliance with health regulations.
Provide home visits as needed during peak demand periods.
Required Qualifications :
Licensure : Current New York State RN License
Certifications : Wound Care Nurse or Wound Ostomy Care Nurse Certification
Education : Bachelor's Degree in Nursing (BSN) required
Experience : Minimum 2 years in a Certified Home Health Agency or post-acute care setting; 2 years in public / community health and medical / surgical nursing
Technical Skills : Proficiency in Epic H2O Hyperspace, Epic H2O Remote Client, and Microsoft Office Suite
Equipment Experience : Smart phone, laptop, desktop computer, multifunction copier
COVID-19 Requirements : Candidates must provide proof of one of the following :
Primary COVID vaccine series plus at least one booster.
Bivalent COVID vaccine (offered from September 2023).
2023-2024 Monovalent COVID vaccine.
