RN Case Manager - Home Health Experience

• Develop and implement patient Plans of Care (POCs) that address the unique needs and goals of each patient.
• Collaborate with patients, physicians, and other team members to ensure seamless coordination of care and optimal health outcomes.
• Document all patient interactions and care delivered in accordance with regulatory requirements and company policies.
• Participate in ongoing education and training to stay current on best practices and industry developments.
