Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality patient care in the comfort of patients' homes, ensuring their health and well-being through comprehensive assessments and personalized treatment plans. This role requires a strong understanding of various patient demographics, including geriatrics and pediatrics, as well as experience in critical care settings.
Duties
- Conduct thorough patient assessments to determine individual healthcare needs.
- Develop and implement personalized care plans based on diagnostic evaluations.
- Administer medications and treatments as prescribed, ensuring adherence to HIPAA regulations.
- Monitor patients' vital signs and overall health status, making adjustments to care plans as necessary.
- Provide education and support to patients and their families regarding health management and disease prevention.
- Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
- Maintain accurate documentation of patient progress and changes in condition.
Experience
- Current Registered Nurse (RN) license in good standing.
- Experience in inpatient settings preferred, with a strong background in critical care.
- Familiarity with pediatric and geriatric patient care is highly desirable.
- Proven ability to perform diagnostic evaluations effectively.
- Strong communication skills with a focus on patient service and empathy.
- Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality.
