Job Description
We are seeking a Registered Nurse Case Manager Inpatient Rehab Hosp to join our team at CarePartners. As a case manager, you will ensure high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes. You will provide, coordinate, and direct care specific to the needs of each rehabilitation patient, collaborating with the rehabilitation program director and facility case management director.
Required Skills and Qualifications
- Bachelor's Degree in Nursing or equivalent experience
- Current licensure as a registered nurse in the state of North Carolina
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
