Job Type: Full-time, Rotating On-Call Weekends

Weekly Salary: $700
We are seeking a compassionate, organized, and detail-oriented Client Care Coordinator to join our team. In this role, you will be responsible for ensuring that our clients receive exceptional care and support throughout their service experience. You will act as the main point of contact for clients, coordinating schedules, managing care plans, and collaborating with caregivers to deliver high-quality care.
Key Responsibilities:
- Client Coordination: Serve as the primary liaison between clients, caregivers, and the care team to ensure smooth communication and satisfaction.
- Scheduling & Appointment Management: Schedule appointments, track services, and coordinate caregiver schedules to ensure timely and appropriate care.
- Client Outreach & Support: Regularly check in with clients and their families to assess satisfaction, gather feedback, and address any concerns or changes in care needs.
- Caregiver Outreach & Support: Regularly check in with caregivers to assess satisfaction, gather feedback and address concerns.
- Documentation & Record Keeping: Maintain accurate client records, including care plans, appointments, and communications, to ensure compliance with company policies and regulatory standards.
- On-Call Support: Provide on-call support to clients and caregivers during evening or weekend hours as needed.
Qualifications:
- Strong Communication Skills: Excellent verbal and written communication skills with a professional and empathetic tone.
- Organizational Skills: Strong attention to detail, ability to manage multiple tasks simultaneously, and prioritize effectively.
- Problem-Solving: Ability to think quickly and adapt to changing situations to ensure that clients’ needs are met.
- Technical Skills: Proficient in using scheduling software, email, and basic office tools. Experience with care management software is a plus.
- Compassionate & Client-Centered: A genuine desire to help people, with a focus on providing exceptional care and service to clients and their families.
- Flexibility: Ability to adapt to shifting schedules and client needs.
Preferred Qualifications:
- Prior Caregiving Experience: Experience in caregiving, nursing, or direct patient care is a plus.
- Local Knowledge: Familiarity with the local community and resources is beneficial.
