We are seeking a compassionate, organized, and detail-oriented Client Care Coordinator to join our team. In this role, you will be responsible for ensuring that our clients receive exceptional care and support throughout their service experience. You will act as the main point of contact for clients, coordinating schedules, managing care plans, and collaborating with caregivers to deliver high-quality care.
Key Responsibilities :
- Client Coordination : Serve as the primary liaison between clients, caregivers, and the care team to ensure smooth communication and satisfaction.
- Scheduling & Appointment Management : Schedule appointments, track services, and coordinate caregiver schedules to ensure timely and appropriate care.
- Client Outreach & Support : Regularly check in with clients and their families to assess satisfaction, gather feedback, and address any concerns or changes in care needs.
- Caregiver Outreach & Support : Regularly check in with caregivers to assess satisfaction, gather feedback and address concerns.
- Documentation & Record Keeping : Maintain accurate client records, including care plans, appointments, and communications, to ensure compliance with company policies and regulatory standards.
- On-Call Support : Provide on-call support to clients and caregivers during evening or weekend hours as needed.
