What are the responsibilities and job description for the Community health worker position at CareSouth?
SUMMARY : The Community Health Worker (CHW) will outreach to community members and patients. The CHW will coach, support, and recruit people for CONNECT.
The CHW will act as a liaison between parents and the health center. This position requires a self-starter that needs very little input to accomplish their work in providing services for our patients.
This is a hybrid role where you will be required to work 50% of time on CONNECT and 50% on School Health Center Navigation.
PRIMARY RESPONSIBILITIES :
A.Collaborative Multi-level Obesity Intervention Engaging Underserved Communities Trial (CONNECT) Responsibilities
1.Health Coaching and Patient Support : Helping people learn about healthy eating and exercise and guiding them to make good choices for their health.
2.Recruitment : Enrolling participants in the study.
3.Surveys and Measurements : Asking study participants questions and measuring things like their height, weight, waist circumference, and blood pressure.
4munity Outreach : Reaching out to the community to get more people involved and aware of the study.
5munity Asset Mapping : Working with community members to find and use local resources, like parks and grocery stores, that can help people live healthier lives.
B.School Health Center Navigator Responsibilities
1.Serves as liaison between the parent / guardians and health center
2.Serves as liaison between CareSouth administration and School administration
3.Responsible for obtaining complete and timely registration documents and consents
4.Responsible for providing registration documents and consents to parent / guardian and faculty / staff
5.Marketing school health center services to parents / guardians and faculty / staff
6.Assist with resolving workflow issues and challenges
7.Assess patient flow
8.Other duties as assigned
C.REQUIREMENTS :
1.Associate's degree, Bachelor's degree preferred
3.Public speaking and presentation experience, preferred
4.CHW training from a certified program (e.g., Louisiana Community Health Outreach Network), preferred
5.Must have reliable transportation throughout the working day.
6.Must be able to work weekends and afterhours, as applicable.
7.Must have ability to learn using Electronic Health Records system(s); must have strong computer skills.
8.Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies, and procedures.
