POSITION TITLE: Director of Case Management
REPORTS TO: Chief Nursing Officer
CLASSIFICATION: Non-Exempt
POSITION SUMMARY: The Director Case Manager position is directly responsible for overseeing and administering all case management functions of the organization. This position is a member of the organization's leadership team and reports to the Chief Nursing Officer. The Director of Case Management Services develops, drafts, and reviews organization policy and prepares, reviews, and approves organization reports, assures deliverables of grants are met,t and manages assigned grants. This position represents the organization at meetings with outside entities and members of the public. Plans assigns, and supervises the work of the organization's case management and associated support staff. Maintains availability to ensure optimal supervision and action regarding supervising and overseeing assigned case management duties. Works under minimal supervision with extensive latitude for—the use of initiative and independent judgment.
ESSENTIAL FUNCTIONS:
The following information is considered the definition of essential functions but does not restrict the tasks that may be assigned. The Director of Case Manager may be reassigned duties and responsibilities at any time due to reasonable accommodation or other staffing reasons.
- Directs and coordinates the daily operations of all case management staff assigned to provide services to all patients at the various clinical sites.
- Develops goals for case management staff consistent with the agency's strategic plan. Oversees and develops comprehensive audit reports to evaluate staff compliance with goals, policies, and procedures. Monitors multiple databases to ensure accurate employee data entry. Prepares Completes and maintains reports of all required documentation pertaining to clients as well as reviews and maintenance of medical records as needed.
- Participates in the agency's strategic planning process through the identification of short- and long-term goals. Develops, drafts, reviews and approves agency policy and procedures related to the organization and treatment consistent with agency goals. Implements policy and procedure changes to include training of staff and vendors.
- Screen and assess clients for their appropriateness for organization services and provide referrals for identified needs of the client and crisis intervention when needed and provide case management to coordinate other services and resources to specialized maternal child case management services to clients and their families with needed
- Participate in Maternal Infant Health Coalition and other relevant local community meetings and engage in collective impact and community engagement activities to improve organizational outcomes while developing and maintaining strategic working relationships with key community partners related to maternal and child health priorities.
JOB REQUIREMENTS AND QUALIFICATIONS:
Education:
- Graduated from an accredited School of Nursing and licensed as a Registered Professional Nurse in the state of Missouri
- Current CPR Certificate
- BSN required
- Certification in Case Management is highly preferred
- MSN preferred
