POSITION TITLE:  Community Health Worker           
 
REPORTS TO:  Manager of Population Health  
 
CLASSIFICATION:  Non-Exempt
 
CareSTL Health is a seeking a Community Health Worker for Population Health Department. The frontline public health worker responsible for building trusted relationships and partnerships in the community served while coaching and empowering patients to take an active role in healthy lifestyles.  In addition, the Community Health Worker effectively educates and links patients, families, communities and providers to community-based resources by coordinating with area agencies. 
ESSENTIAL FUNCTIONS:
The following information is considered the definition of essential functions but does not restrict the tasks that may be assigned.  The Community Health Worker may be reassigned duties and responsibilities at any time due to reasonable accommodation or other staffing reasons. 
 
  1. Assist consumers with enrolling in health plan offerings and other program and service initiatives as needed.
  2. Track enrollment and outreach activities and outcomes, and provide monthly reports.
  3. Make outreach calls to health plan members and other patients with a minimum of 75 calls per day.
  4. Organize and attend community events and health fairs as assisgned.
  5. Complete and document Protocol for Responding to and Assessing Patients’ Assets, Risks, and Experiences (PRAPARE) tool for all patients seen and address barriers as well as provide resources to patients who are in need.
 
OTHER FUNCTIONS
  • Empower, coach, and serve as a liaison with the patient, CHC clinical care team, and community partners.
  • Ensure routine adherence to all regulations surrounding HRDA, CMS, MPCA, etc., as well as all laws and regulatory requirements surrounding confidentiality.
  • Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
  • Follow-up with members via phone calls, Telehealth, home visits, and visits to other settings where members can be reached.
  • Connect members with transportation resources and give appointment reminders in special circumstances. Transporting members is strictly prohibited.
  • Performs general support activities and carries out special projects as required.
 
 
JOB REQUIREMENTS AND QUALIFICATIONS:
Education:
  • High School Diploma or GED required
  • Completed Community Health Worker Training Program OR willing to complete Community Health Worker training program within 1 year of starting employment.
 
Experience:
  • Must have 2 years experience working in community health, human services, community outreach and/or healthcare.
  • Desirable experience includes: customer service, patient/peer coaching, medical patient interaction, community relations, outreach, data entry, public speaking, non-profits.

Additional Information:

  • Clinic hours are Monday - Friday 8 a.m. to 5 p.m.
  • 11 paid holidays per year.
