We are seeking a dedicated RN to join our team as a Hospice Case Manager.

Responsibilities:
- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identify patients' physical, psychosocial, and environmental needs, reassessing as needed every 15 days.
- Develop and coordinate individualized plans of care while maintaining accurate clinical notes.
- Collaborate with patients, families, attending physicians, and other IDG members to provide daily care.
Requirements
To be successful in this role, you will need:
- One year of clinical experience.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR certification.
- A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
