What are the responsibilities and job description for the Hospice RN Case Manager Position position at Olive Grove Hospice?
Job Summary:
Hospice RN Case Manager will coordinate care for patients and their families. The ideal candidate is a registered nurse with experience in long term care or homecare/hospice settings, possessing excellent judgment and decision-making abilities.
Nursing Responsibilities:
Coordinate patient services with other members of the IDG as well as contracted services when needed. Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis. Consult with the attending physician concerning changes in patients' condition requiring order changes and/or changes in the plan of care. Submit complete, accurate and relevant clinical notes in a timely manner. Participate in IDG and case conferences. Take responsibility for ensuring patients' plans of care are kept current. Perform on-call duties per policy and agency need. Conduct initial as well as ongoing comprehensive assessments of the patients. Supervise and evaluate the care given by the Hospice Aide at a minimum of once every 14 days and as needed. Graduate of an accredited school of nursing. Must be registered and licensed with the State of working as a RN, and must maintain current unencumbered license with the Board of Nursing.
