Responsibilities:
The Registered Nurse Case Manager assumes full nursing responsibility for delivering high-quality patient care, continuously evaluating performance to increase productivity and quality of care.
- Deliver the Plan of Care for each hospice patient
- Evaluate personal and professional performance to improve care
- Adhere to and support hospice policies/procedures/goals/objectives to ensure quality patient care
- Maintain independence and sound critical decision-making regarding patient care
- Develop and implement treatment strategies promoting physical, psychosocial, emotional, and spiritual well-being
Requirements:
Experience:
- One year of clinical experience
Licenses/Certifications:
- Current RN licensure in the state of practice and one year of clinical experience
- Current CPR Certification
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
