About the Role:
- You will be responsible for coordinating total plan of care with other healthcare professionals involved in care and helping to achieve and maintain continuity of patient care.
- You will make the initial and/or comprehensive nursing evaluation visit, determine primary focus of care, develop the plan of care within state specific guidelines, and submit accurate, complete, and timely documentation in collaboration with physician orders.
Your Skills and Qualifications:
- Current RN licensure in state of practice
