Overview
We are seeking a compassionate and dedicated Hospice Nurse to join our team. In this vital role, you will provide high-quality nursing care to patients with terminal illnesses, ensuring their comfort and dignity during their final stages of life. As a Hospice Nurse, you will work closely with patients, families, and interdisciplinary teams to deliver holistic care that addresses physical, emotional, and spiritual needs.
CARE PLAN
Establish a Care Plan to meet their needs, implement interventions and evaluate the patients and family's response to the Care Plan to provide appropriate comprehensive and responsive care plan
Responsibilities:
- Assess and admit patients for Hospice care
- Plan, coordinate, and implement health services for new patients
- Provide clinical oversight of medications and plan of care
- Review clinical documentation for consistency and accuracy
- Deliver Explanation of Benefits (EOB's) to patients
- Advanced Care Planning
- Provide director support and general clerical duties related to position
Location
LANCASTER
Duties
- Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
- Administer medications and treatments in accordance with established standards and protocols.
- Provide palliative care, focusing on pain management and symptom relief for patients in hospice settings.
- Collaborate with physicians and other healthcare professionals to coordinate patient care.
- Educate patients and families about disease processes, treatment options, and end-of-life care.
- Maintain accurate documentation using eClinicalWorks and ensure compliance with ICD-10 coding standards.
- Participate in memory care activities as needed to support cognitive function in patients.
- Ensure adherence to sterile processing techniques during any necessary procedures or treatments.
- Offer emotional support to patients and families, addressing concerns related to hospice palliative medicine.
Salary: Starting Wage $100 (wage depends on type of visit, experience, and distance from patient)
Experience
- Current nursing license with experience in hospice or palliative care preferred.
- Familiarity with ventilate standards and esthetic laser treatment is a plus.
- Strong skills in patient assessment and the ability to work effectively within an interdisciplinary team.
- Excellent communication skills for interacting with patients, families, and healthcare professionals.
- Ability to demonstrate compassion, empathy, and patience in challenging situations.
- At least 1 year experience in Hospice
Preferred Qualifications:
- Current California RN license
- Understands and embraces philosophy of hospice and end-of-life. Also understands needs of the terminally ill.
- Good prioritization and communication skills.
- EMR: HospiceMD
- Valid CA Driver’s License and auto insurance
- Current CPR/BLS
- Current TB/Physical
- Pass Background Clearance
