General Summary:
The Admission Nurse is responsible for conducting evaluation and information visits to potential patients and their families, obtaining consents, election forms, performing an initial comprehensive physical assessment and developing a plan of care. This is a FULL TIME position.
The Admission Nurse participates as a member of the IDG, actively coordinating all aspects of the patient’s hospice care, in accordance with current professional standards and practice, including participating in ongoing interdisciplinary comprehensive assessments, developing and evaluating the plan of care and contributing to patient and family counseling and education.

Admission Process:
- Explain the hospice program philosophy and services to prospective patients and their family.
- Evaluate the patient’s medical history, physical status, and functional needs.
- Consult with and make a recommendation to the Medical Director/Hospice Physician regarding appropriateness of admission based on the criteria established by the hospice and the payer source.
- Obtain consents and benefit elections as appropriate, complete all admission paperwork.
- Complete (at a minimum) an initial assessment, in most cases complete a comprehensive assessment, of patient and family needs in coordination with the attending physician, Hospice Medical Director/Hospice Physician, and other members of the interdisciplinary team to develop the Plan of Care.
- Provide direct care and nursing intervention as dictated by physician order, patient need and the Plan of Care
- Communicate with the referral source, long term care facility, insurance company and/or others as appropriate regarding admission status of patient/family.
- Provide appropriate patient/family/caregiver teaching and instruction.
Administrative:
- Record clinical findings and services in the medical record.
- Coordinate all patient care activities including ordering/obtaining pharmaceuticals, durable medical equipment and supplies as indicated in the Plan of Care.
- Attend and participate in IDG meetings.
- Participate in Quality Assessment / Performance Improvement activities.
- Attend and participate in continuing education / inservice activities.
