We are seeking a dedicated Family Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive primary care services to patients of all ages, with a focus on family-centered care.
Position Responsibilities
Clinical
A. Obtains patient histories on ED patients.
B. Performs physical assessments pertinent to patients' presenting complaint.
C. Evaluates assessment data and establishes diagnoses.
D. Differentiates patient care situations which require immediate and non-immediate actions and responds accordingly.
E. Orders and evaluates appropriate diagnostic studies.
F. Performs and monitors therapeutic procedures.
G. Assesses and/or manages follow-up plans.
H. Records and documents assessment data, interventions, results, and patient outcomes and referrals.
I. Educates patients and/or families in order to promote wellness, prevent health problems, maintain current health and intervene in acute or chronic illness.
J. Documents discharge teaching and patient instructions for follow-up.
Administrative
A. Facilitates and implements systems which promote and maintain effective communication and collaboration with inter/intra hospital physicians, nurses and other members of the health care team.
B. Maintains appropriate records of clinical and educational activities.
C. Collaborates with physicians in the development of clinical guidelines and provides suitable references for the delivery of patient care.
D. Demonstrates ability to work effectively with patients; families and with professional and support personnel who provide patient care.
E. Recognizes and uses appropriate channels to communicate situations of concern involving patients, family members, physicians or other staff members.
F. Collaborates with physicians to reach consensual agreement on staffing schedules.
G. Must be an active member of the medical staff.
H. Co-Leads the Antimicrobial Stewardship Program and is responsible for assisting in developing, implementing, maintaining the Antimicrobial Stewardship Program in compliance with DNV and the CMS Condition of Participation and improving overall antibiotic use.
Research/Scholarly Activities
A. Identifies the need for and participates in appropriate scholarly/research activities.
B. Utilizes theories of evidence-based medicine to guide practice.
Education
A. Acts as a resource to collaborating staff and contributes to staff education for improvement of clinical skills, patient care and knowledge base.
B. Maintains and enhances current level of knowledge relative to professional practice, as well as continuing education requirements necessary for licensure and prescriptive privileges.
C. Accepts self-responsibility for advancing skills and continuing education by:
1. Setting realistic and measurable goals for individual developmental needs.
2. Assessing performance status individually and within established process of partner group.
3. Seeking learning experiences to strengthen skill and knowledge development and to maintain awareness of current developments in nursing.
