Carillon at Belleview Station is looking for a Coordinator to implement our new mild cognitive impairment program called Bridge to Belonging. This program is targeted to help those residents in Assisted Living who are starting to experience some cognitive decline, but are not a good fit for memory care at this time. The Coordinator will be responsible for running the program day to day, partnering close with other team members to facilitate and develop this program. This is a full time position, must have weekend availability.

Duties and Responsibilities:
- Facilitate the day to day Mild Cognitive Impairment program
- Assist in the preparation, posting, and distribution of a customized monthly newsletter and calendar of the Mild Cognitive Impairment program and events in a timely manner.
- Collaborate with a team of program coordinators to share best practices and innovation in cognitive stimulation programming.
- Participate in a new resident welcome program and obtain background and interests of each new Mild Cognitive Impairment program resident through formal interview and assessment.
- Encourage resident and family involvement in planning the monthly calendar of events and participating in scheduled activities.
- Provide monthly reports to the Area Director of Memory Care on the efficacy of the program, and make recommendations for improvement
- Ensure updated documentation in our electronic records system ALIS, including updating the communication portal with the family weekly.
- Participate in care conferences for residents in the Mild Cognitive Impairment Program.
- Assists in overall management of expenditures within budgeted allocations.
Qualifications:
- Minimum one year working with people with Dementia required
- Activity Professional Certification preferred
- Due to the nature of the work performed by Ascent Living Communities, and that this position works with a vulnerable population, we do require a criminal background check be completed
Pay: $23.50 an hour
