ROLE RESPONSIBILITIES: The Client Care Coordinator is dedicated to compliance, case management, quality care, care coordination and evaluation, scheduling, and following highly-organized and detail-oriented processes. Your duties will include the following:
- Provide excellent record-keeping and information management
- Coordinate and evaluate care and services to Clients
- Cultivate a hospitable, welcoming, solution-oriented approach to care services
- Provide ongoing case management and support
- Provide next level customer service, assisting customers with a professional, welcoming demeanor
- Functioning as a right hand assistant to the Executive Director
- Maintain complete and organized files and records
- Conduct ongoing Client satisfaction, service, and care evaluation by person and via phone
- Provide continual support to Office
- Optimizing service hours and scheduling Caregivers and Clients as needed
- Assist with events and orientations
ROLE QUALIFICATIONS

- Have excellent written and oral communication articulation in English
- Demonstrate accuracy and attention to detail, maintaining accurate records and documentation

- Demonstrate strong commitment to compliance
- Knowledge of best practices for handling PHI in compliance with HIPAA regulations
- Bachelor's Degree Preferred
