Duties:
- Provide direct patient care in their home
- Administer medications and treatments as prescribed by healthcare providers.
- Monitor patients' vital signs and report any changes or abnormalities.
- Assist with diagnostic procedures such as spinal taps and catheter care.
- Collaborate with other healthcare professionals to develop and implement patient care plans.
- Educate patients and their families on health conditions, medications, and self-care techniques.
- Document patient information accurately and maintain medical records.
Requirements:
- Must be a licensed practical nurse (LPN) with a valid state license.
- Previous experience working in a hospital, nursing home, primary care clinic, or medical office is preferred.
- Knowledge and experience in working with individuals with developmental disabilities is a plus.
- Strong communication skills and the ability to work well in a team environment.
- Excellent organizational skills and attention to detail.
