Job Overview
We are seeking a dynamic and motivated Hospice Clinical Liaison (LVN) to join our exceptional team. In this role, you will be responsible for building and maintaining strong relationships within the community to promote our hospice services. The ideal candidate will possess a blend of sales and clinical experience and excellent interpersonal skills, enabling them to effectively engage with diverse audiences. This position requires a proactive approach to outreach, networking, and collaboration with local organizations and referral sources.
Responsibilities
- Educate and guide patients and families through the hospice admission process, ensuring informed decision-making and a smooth transition to care.
- Evaluate patients for potential hospice appropriateness as ordered by the physician.
- Develop and implement community outreach strategies to enhance visibility and engagement.
- Assist with patient intake and completion of required forms
- Conduct presentations to potential clients and community groups.
- Organize/support education activities with referral sources
- Collaborate with local businesses and organizations to foster partnerships that benefit the community.
- Attend community events and networking functions to promote our company.
- Provide insights on market trends and customer feedback to inform service development and marketing strategies.
- Negotiate contracts and agreements with community partners as needed.
- Foster and maintain positive relationships with existing and prospective referral and payer sources
- Maintain a thorough understanding of community resources and support patients in accessing these resources when Caring Hearts of Texas services are not available.

QUALIFICATIONS:
- Graduation from an accredited LVN school.
- Current LVN license issued by the State of Texas
- Must maintain current CPR/First Aid training.
- Excellent verbal, written, and interpersonal communication skills.
- Proficient in telephone techniques including phone etiquette and handling of calls.
- Ability to work as a team player and multi-task.
- Reliable means of transportation.
- At least 2 years of hospice experience
- , preferably with an established referral network.
