What are the responsibilities and job description for the Geriatric Care Manager/Nurse Case Manager position at Caring Strategies?

Caring Strategies Care Management has been enriching the lives of older adults since 2007. We provide expert guidance and consultation to older adults and their adult children in making complex healthcare and long term care decisions. We take great pride in identifying customized solutions and resources carefully selected for each client’s personalized needs, based on our comprehensive assessment findings.
We are looking for a part time LPN or RN who has geriatric and dementia experience and possesses a compassionate personality and ability to problem solve. Case management or hospice experience is a plus.

- Attention to detail is also a must.
- Duties include in-home assessments, management, and coordination of medical care for seniors, liaison with health care providers and families; education/ counseling of families and more.
Care Management Responsibilities include:
- Meet and assess clients in their home setting to gather information and assess in a non-threatening manner. Assessment includes medical, cognitive, emotional, functional and safety components with an understanding of ability to afford recommended resources.
- Manage and coordinate all aspects of care to maximize the independence, health, and well-being of our older adult clients, including accompanying them to physician appointments or ER visits.
- Create written summaries of findings and recommendations for client’s responsible party.
- Implement and monitor all aspects of the care plan, including following through on action items, identifying obstacles to follow through and updating and modifying goals as needed.
- Navigate complex family and professional dynamics to overcome challenges and achieve positive outcomes for clients.
- Document services provided in a timely and thorough manner.
- Provide on-call services as part of an on-call team rotation.
- Contribute to the overall support and functioning of the Care Management team, through active participation in Care Management meetings and providing coverage of cases as needed.
- Provide billable services to clients at 90% of total work time after initial 3-month training period.
- Educate professionals and families on who we are and how we can help their clients/family members.
- When not with a client, work is largely conducted from home, although you may choose to work from our office in south Huntsville.
- Please note that most of our clients live near downtown Huntsville/Jones Valley areas
Knowledge, Skills and Abilities
- Must have a heart for elders and a strong desire to make their lives better
- Integrity, honesty, and commitment to high standards of excellence and professional ethics
- Knowledge and experience with the aging process and with cognitive changes and other conditions affecting older adults
- Highly skilled in formulating holistic assessments and developing care plans that incorporate effective use of resources, creative problem-solving, and sound care management judgment
- Excellent interpersonal skills and ability to connect with a wide range of individuals, including physicians, attorneys, and trust officers
- Knowledge of the aging community including levels of care, cost, and state regulations
- Strong analytical and problem-solving skills
- Strong verbal and written communication skills
- Experience working with virtual platforms including Zoom, Facetime
- Impeccable attention to detail and follow through in completing communication
- Home office capabilities required including scanner/printer and current smartphone
- A valid driver’s license, proof of insurance, and safe driving record. Must be comfortable with background checks
- A minimum of two references from existing or prior employers
Job Type: Part-time
Pay: From $30.00 per hour
Expected hours: 20 – 32 per week
