Licensed Social Worker (LSW) - part time
Care for the social and emotional needs of the aging population through end of life by connecting patients and their families to home health and hospice programs and resources. Collaborate with the interdisciplinary team. Make a lasting difference in patients and families lives. Counsel patients and families on stress management. Provide crisis intervention. Design and implement patient treatment plans. Attend regularly scheduled interdisciplinary group meetings. Assist the team in recognizing the psychological stress of a terminal illness. Identify and connect patients with community resources. Additionally, oversee the hospice volunteer program.
What you will need:
· Must have LSW in good standing with the state of employment- required
· CPR certification, we will provide if needed
