What are the responsibilities and job description for the Clinical Care Coordinator RN position at Carle Health System?
We're Looking for an Experienced Registered Nurse:
To join our team as an Ambulatory Nurse, you will utilize a systematic approach to nursing by incorporating assessment, planning, implementation and evaluation in the practice of nursing.
You will provide healthcare to individuals, families, and communities within the RN scope of practice as defined by the Illinois Board of Nursing.
Your Key Responsibilities Will Include:
- Providing and facilitating competent health care of patients and families in area of practice
- Ensuring adherence to established standards of care and scope of practice
- Ensuring documentation of care is complete and accurate
- Providing health and wellness education to individuals and groups
- Acting as preceptor and supporting the development of other staff and healthcare students
- Providing leadership for initiatives designed to improve the quality of care or clinical outcomes
- Assisting Nursing Supervisor/coordinator in the development of departmental standing orders
- Telephone triage and management of items on the worklist
- Assist with procedures
- Patient education
- Management of a nurse schedule for patients needing injections, suture removal, etc
- Wound care, catheter insertions, IV starts
- Assist providers with the day-to-day patient flow
