Position Overview
We are seeking a Psychiatric Nurse (LPN) to join our Assertive Community Treatment (ACT) Team. The ACT Nurse will provide clinical services to all clients receiving ACT team services and oversee the medical and clinical aspects of service delivery.
This role includes medication monitoring, service coordination, crisis intervention, and collaborating with the ACT team to ensure optimal client care. You will work directly with clients, families, and other healthcare providers to deliver tailored treatment plans and ensure individuals receive the resources and support they need.
Key Responsibilities
- Monitor and oversee all medical/clinical aspects of services delivered.
- Provide medication monitoring and documentation.
- Deliver psychopharmacologic and medical services within your scope of practice.
- Collaborate with ACT team members to monitor clinical status and response to treatment.
- Conduct crisis assessments, symptom management, and intervention planning.
- Provide individual counseling, substance abuse treatment, and psycho-educational support.
- Assist in creating transition plans when clients move in and out of services.
- Act as a consultant to primary healthcare providers and community medical staff.
- Ensure proper documentation, including treatment plans and progress notes.
- Be available for crisis response and support to the ACT team on a 24/7/365 basis.
Qualifications

Education & Licensure:
- Active LPN license in North Carolina.
- Each ACT team member must complete DMH-approved ACTT training.

Experience:
- Must demonstrate competence in providing ACT services prior to service provision.

Certifications & Training:
- NVCI certification required.
- Training in population served, client rights, and confidentiality.
- Valid driver’s license and current automobile insurance required.

Skills:
- Ability to organize and manage time effectively.
- Excellent oral and written communication skills.
- Knowledge of evidence-based practices, service standards, and care policies.
- Competent with office software applications.
- Strong problem-solving and client-focused approach.
Physical Requirements
- Light work; must be able to lift no more than 20 pounds.
- Must be free from all illicit substance use.
