What are the responsibilities and job description for the ACT Team Nurse position at Carter Clinic, P.A.?
Well-established and diverse behavioral health practice seeking to add nurse to ACT Team. Duties include:
1. Conducts intake interviews of new consumers; records health information, family history and reason for referral; interviews patient and, as necessary, others to obtain information needed in diagnosis, treatment and supervision.
2. Dispenses and administers medications as prescribed, consults with physician regarding consumer response and case history, councils consumer regarding use and abuse of medications, assists in establishing procedures for obtaining and returning medications, interventions and orders medications, may be involved in the use, troubleshooting and training medical, nursing and technician staff on the use of medication dispensing unit.
3. Participates in the assessment of consumers needs with doctors, social workers and other members of a mental health treatment team, consults with family members, teachers, probation officers and representatives of other agencies on diagnostic treatment and case disposition matters.
4. Monitors patient’s general health status; obtains and records vitals; performs various screening and diagnostic tests; obtains, submits, and reviews results of laboratory tests.
5. Participates in individual, group and/or family treatment sessions; conducts brief spontaneous individual, group and/or family treatment sessions; conducts brief, spontaneous individual counseling to patients and family members; encourages physical health by teaching consumers on various health and safety related issues.
6. Plans, develops and implements program designed for consumer education and rehabilitation; provides follow-up on medical needs.
7. Prepares reports for representatives of support services agencies to assist consumers and their families in obtaining needed services; prepares consumers’ progress notes and summaries.
8. Performs various related tasks, documents and reports necessary information to police or probation officers, completes required forms and documents, demonstrates desired behaviors and communication techniques.
9. Serves as the primary monitor of his or her consumers’ functioning and communicates that to other staff.
10. Assists in connecting the consumer to appropriate social networks with other team members.
11. Provides much of direct contact for his or her consumers, such as accompanying them to appointments and helps them with daily living activities (apartment upkeep, shopping laundry etc).
12. Assists with any other activities that are necessary to maintain psychiatric stability in a community-based setting.
B. Crisis Duties
1. Provide support, services and treatments necessary to provide integrated crisis response, crisis stabilization interventions, and crisis prevention activities.
2. Provide immediate telephonic response to crisis calls to assess the crisis and determine the risk, mental status, medical stability and appropriate response.
3. Provide immediate evaluation, triage and access to acute mental health, developmental disabilities and/or substance abuse services, treatment and supports to effect symptom reduction, harm reduction and/or to safely transition persons in acute crises to appropriate crisis stabilization and detoxification supports/services.
4. Provide face-to-face crisis interventions as needed.
