Carolina SeniorCare - New Bern is a new center in the process of becoming a Program of All Inclusive Care for the Elderly (PACE). Nurse Position Summary: The Registered Nurse (RN) provides direct participant care in the provision of nursing services to participants of the PACE center. The RN is responsible for meeting all standards associated with the provision of nursing care in accordance with applicable federal and state regulations, licensure law, agency policies, procedure and standards.

- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Must have a valid driver’s license, proof of insurance and have means of transportation. Must be able to meet Carolina SeniorCare's auto insurance carrier's standards for coverage.
- Education:
- Graduate of an accredited school of nursing. Current North Carolina Registered Nurse (RN) Licensure. Bachelor’s Degree of Science in Nursing (BSN) is preferred. Current Cardio Pulmonary Resuscitation (CPR) certification required. Current First Aid Certification required or ability to obtain.
- Experience:
- A minimum of one year’s experience in working with the frail or elderly population required.
- At least one year of experience in long-term care, acute care, primary care, home care or PACE nursing preferred.

- Ability to react calmly and effectively in emergency situations.

- Performs nursing skills knowledgeably and competently. Practices within the Registered Nurse scope of practice regulated by the North Carolina Board of Nursing.
- Follows established Standards of Nursing Practice. Implements and demonstrates ongoing adherence to Carolina SeniorCare policies and procedures.
- Under the direction of the Clinical Supervisor, conducts an initial comprehensive nursing assessment in-person with potential enrollees as needed. Documents assessment findings and collaborates with the nurse care manager to develop a plan of care for the prospective participant.
- Under the direction of the Clinical Supervisor, conducts an in-person nursing reassessment of enrolled participants on behalf of assigned nurse care manager as needed. Documents assessment findings within participant’s medical record.
- Provides assessment of participants, monitors health problems and health status as needed, and implements nursing interventions. Reports a participant’s condition and response to medication and treatment to primary care provider and/or nurse care manager.
- Participates in development of plan to meet personal care needs as necessary, and assists with setting long-term and short-term goals.
- Under the direction of the Clinical Supervisor, oversees scheduling of clinic appointments and ensures priority is given to participants based on acuity of medical condition. Promotes a timely flow of participants during hours of clinic operation.
- Conducts and responds to triage of incoming clinic telephone calls. Assesses participants with acute needs telephonically to the extent possible.
- Implements orders written by the clinic’s primary care providers.
- Notifies the primary care provider of significant change(s) in a participant’s condition. Provides emergency care, including arranging for transportation.
- Administers medications to participants as prescribed via oral, inhalation, intradermal, transdermal, subcutaneous, intramuscular, intravenous, rectal, ophthalmic, and otic routes. Documents medication administration on corresponding medication administration record (MAR). Evaluates medication effectiveness as needed.
- Coordinates dispensing of requested medication refills with contracted pharmacy staff.
- Assists with collection and preparation of lab specimens for analysis by contracted laboratory vendor. Performs in-house analysis of CLIA-waived lab tests and communicates results to primary care provider.
- Handles potentially infectious specimens and equipment with appropriate biohazard precautions and adheres to standard precautions.
- Assists with monitoring of participants who are prescribed anticoagulant therapy. Notifies primary care provider timely of pertinent lab results and conveys instructions regarding prescribed anticoagulant dosage titration to participant/caregiver.
- Performs wound care as ordered by clinic providers. Completes nursing assessment of wounds and documents assessment findings in medical record.
- Performs timely assessment of participants who sustain unanticipated incidents while attending the Day Center.
- Communicates effectively in participant, family, and clinic meetings.
- Assists with completion of paperwork for external specialist appointments and informs nurse care manager.
- Educates participants and families regarding medication and treatment instructions.
- Ensures that exam rooms are stocked with necessary supplies and equipment. Ensures cleanliness of clinic department.
- Develops and implements an individualized “self-administration of medications” education program for participants as needed to promote independence and adherence to medication regimen.
- Provides on-call coverage on a rotating basis with other clinical staff to troubleshoot, advise, and coordinate participant care.
- Delegates tasks appropriately to unlicensed assistive personnel to promote efficiency.
- Participates in and supports internal Quality Assurance and Performance Improvement (QAPI) initiatives. Assists with collection of QAPI data upon request.
- Demonstrates positive interpersonal relations in dealing with fellow employees, supervisors, participants, and families of participants.
- Maintains a safe, clean clinic environment. Reports potential hazards to appropriate staff in a timely manner. Adheres to Occupational Safety and Health Administration (OSHA) safety guidelines.
- Maintains current documentation within participant medical records that is compliant with regulatory standards.
- Conducts home visits as needed to assess participants or to provide nursing care.
- Demonstrates understanding of participants as unique individuals in need of compassionate, supportive, emotional, physical, and spiritual care.
- Protects privacy and maintains confidentiality of all health information of participants and employees, as well as all organization business.
- Ensures participant privacy during delivery of care and during interactions with participants.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional licensure and any required certifications.
- Performs other duties as required or requested by Clinical Supervisor.
