Position Summary

Carpenter's Place is a faith-based, nonprofit organization whose mission is to provide the tools necessary for rebuilding the lives of the homeless and families at risk. Through case management and comprehensive supportive services, the Housing Case Manager supports unhoused adults to secure and maintain housing within Carpenter's Place various housing programs.
Key Responsibilities:
Housing Case Management
- Provide advocacy and case management services to a caseload of participants in transitional housing and scattered-site permanent supportive housing.
- Build rapport with Guests, conducting persistent and proactive outreach to program participants as necessary to ensure effective service delivery of case management and other supportive services.
- Develop a mutually agreeable personalized life recovery plan with participants and connect with appropriate services to support participants in reaching goals.
- Maintain regular face to face meetings with housing participants to monitor progress towards achieving life recovery goals within the established timeframe.
- Facilitate enrollment in benefit programs, including but not limited to: Medicare, Medicaid, Link, SSI, SSDI, etc.
- Provide information and assistance with enrollment in benefit programs, including but not limited to: Medicare, Medicaid, Link, SSI, SSDI, etc.
- Provide information and referrals to connect participants with program and services.
- Coordinate with the Dayroom, Adult Case Management, Family Services Case Management, Employment and Education Case Management, and other Carpenter's Place programs and services to support engagement of housing participants with other services as needed.
- Complete housing inspections on regularly scheduled intervals. Report all maintenance and safety issues with housing units to the appropriate building manager and/or Carpenter's Place Administrator.
- Teach life skills to facilitate successful residency within housing programs and promote self-sufficiency.
- Facilitate activities, house meetings, community outings and engagement to support self-sufficiency.
- Provide transportation for program participants on an as needed basis.
- Follow up on incident reports involving participants, verbally de-escalate situations, and respond to crisis needs of housing programs participants.
Housing Coordination
- Recruit, identify potential participants, and follow up on housing referrals in a timely manner to ensure full occupancy of housing units.
- Conduct intake interviews, assessments, and orientations to determine eligibility for housing programs.
- Coordinate and assist with Guest move-ins and move-outs.
- Assist housing participants residing within our transitional housing programs to identify and transition to permanent stable housing solutions by the end of their eligibility period, addressing health, safety, and welfare needs.
- Collect program fees for housing programs monthly and submit timely to the finance department.
- Notify the building manager of participants that move in and out of transitional housing programs.
- Ensure apartments are ready for new occupancy once a participant exits the program.
Other Duties
- Ensure accurate and timely completion of appropriate paperwork and data entry (case notes, service plans, housing intakes and assessments).
- Complete monthly and reporting forms as required by grants and the organization.
- Crosstrain to provide back-up support for Women's & Families' Transitional Housing programs.
- All other duties as may be relevant to the role may be assigned.
Qualifications Education Level
- Bachelor's degree in social work, human services, psychology, or related field.
Experience Requirement
- Minimum of 2 years providing case management or housing-based supportive services.
Technical Skills
- Accurately identify a client's complex needs, including but not limited to employment, medical, housing, financial, and mental health needs, to determine the most appropriate services.
- Coordination of services across the social services system.
- Effectively present program information to businesses, organizations, and key stakeholders to secure engagement and support.
- Ability to de-escalate Guest conflict through active listening, demonstrating empathy, and using non-confrontational communication.
- Ability to multi-task and remain calm under pressure.
- Ability to set professional boundaries with clients and enforce dayroom rules and program policies.
- Ability to demonstrate a trauma informed and culturally humble response to Guests circumstances.
Compensation & Benefits:
- Paying from $20.00 - $23.00 per hour
- The position is full-time, offering 40 hours of work per week. This is an hourly, non-exempt position.

Essential Functions
Able to pass a background check and drug test
Attendance and Punctuality
Punctuality and daily attendance are critical to this role. Normal hours of operation for this position are Monday - Thursday from 7:30 AM - 4:30 PM and Fridays 8:00 AM to 2:30 PM.
Computer Skills
Proficiency in Microsoft Office, including Outlook, Word and Excel. Ability to learn Mission Tracker and/or other required software such as but not limited to online conferencing software, HMIS, etc.
Certificates, Licenses, Registrations
Must possess a valid driver's license, reliable vehicle, and verify insurance for fieldwork.
