Position Purpose :
The Supported Housing Case Manager works with clients residing in the Supported Housing buildings to provide behavioral health treatment in the client's natural setting. The case manager is responsible for completing comprehensive assessments, developing an individual service plan, and monitoring progress. The case manager provides weekly case management and community support services both on site and within the community. Staff assist clients with fostering independence and maintaining safety and stability while maintaining housing and benefits and improving self-determination through illness management. The case manager coordinates care with the client, family / natural supports, and outside providers. Staff attends weekly client staffing, monthly meetings with the program managers, and monitors and reports on performance indicators and grant deliverables.
Job Requirements :
- Bachelor’s Degree in health related field, two years experience in mental health preferred
- Valid driver’s license and reliable transportation
