We are dedicated to giving the time and training needed for you to successfully meet the needs of our patients in this 60-bed skilled nursing facility.
The purpose of this position is to provide services that meet the social and/or emotional needs affecting the resident's ability to achieve their highest level of function; participate in the development of resident comprehensive care plans; develop policies and procedures to provide social services to residents in compliance with federal, state and local regulations.
Qualifications:
- License or certification by the state as a social worker or
- Minimum of bachelor's degree in Social Work or a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling and psychology and
- 1 year of supervised social work experience in a health care setting, working directly with individuals.
