Qualifications
Associates or higher degree in healthcare or human services. Bachelor’s degree in Social Work preferred. One year prior relevant work experience (i.e. Discharge planning, care coordination). Must be proficient with computer software programs. Great organizational and time management skills. Effective verbal and written communication skills.
