CHMC – Case Management- Social Services Coordinator – Per Diem -Day Shift-8:00a-4p- Requisition 8031
Pay Range: $26.40-$34.31*
The social services coordinator evaluates patient’s ability to progress throughout the continuum of care. Works collaboratively with providers, nursing and other member of the multidisciplinary team to effect timely and appropriate patient management. Responsibilities include taking the lead on patients with complex discharge or psychosocial needs, readmissions, and high users. Meets with patient and family in developing the discharge plan; documents plan. Facilitates family health care decision making. Consults with ED Case Manager and Provider about complex discharge planning cases, discusses anticipated plan of care, discharge plan or alternative services. Initiates referrals to community resources, long term care facilities, SNF, hospice, and palliative care. Set up equipment, home health referrals, community services, etc. Provides crisis intervention, counseling and emotional support.
Qualifications: Associates or higher degree in healthcare or human services. Bachelor’s degree in Social Work preferred. One year prior relevant work experience (i.e. Discharge planning, care coordination). Must be proficient with computer software programs. Great organizational and time management skills. Effective verbal and written communication skills.
