Job Summary
We are seeking a dedicated and organized Care Support Coordinator to join our team. The ideal candidate will play a crucial role in delivering case management services to victims and survivors of domestic violence and sexual assault, including their children, both in the shelter and in the community. This role is centered on a whole-person care approach to empower victims and survivors to attain independence through access to supportive resources. The Care Support Coordinator is responsible for conducting weekly meetings with each program participant to advance their care plan in collaboration with them. This position requires strong interpersonal skills, reliability and consistency in attendance and performance, attention to detail, and the ability to manage multiple responsibilities efficiently. A background in case management, housing navigation, social services, or healthcare, especially within Medi-Cal or CalAIM programs is preferred.
Responsibilities
- Conduct comprehensive initial assessments and ongoing evaluations with program participants to identify their medical, legal, social services, safety, and housing needs.
- Develop and update individualized care plans based on participant goals, ensuring a clear understanding of potential outcomes and success benchmarks.
- Facilitate regular meetings to support participants' progress, celebrate successes, and adjust goals as necessary.
- Maintain accurate and clinically appropriate documentation of client interactions for billing purposes.
- Assist participants in completing California Victim Compensation (CalVCB) and housing applications, supporting re-housing, credit repair, safety planning, and grievance processes as needed.
- Facilitate program participant moves to secure housing, overseeing coordination, funding, and the logistics of moving.
- Maintain relationships with community partners, including schools, faith-based organizations, property managers, and social service agencies, to ensure comprehensive support for program participants.
- Stay informed about best practices in domestic and sexual violence services and available community resources, particularly for those with acute medical and behavioral health needs.
- Provide crisis intervention, emotional support, and peer counseling using a trauma-informed and culturally appropriate approach.
- Educate community members about domestic violence, its health impacts, and available resources, including medical services, shelters, and legal assistance.
- Encourage participant independence and health improvement by helping them understand their health conditions, linking them to healthcare resources, and fostering social capital for healthier lifestyles.
- Maintain accurate electronic records and documentation of services, including case notes, outcome records, and required program documentation.
- Participate in team meetings and submit reports as requested by the Director of Programs.
- Provide mobile advocacy to ensure equitable access to services, meeting participants in safe, mutually agreed-upon locations.
- Offer advocacy and case management support throughout the program, following participants for at least six months after securing permanent housing.
Requirements
- Bachelor’s degree in a relevant field or equivalent professional experience in human services or a related area.
- Knowledge of violence dynamics and familiarity with resources in Yuba, Sutter, and Colusa Counties for homeless and underserved populations, with experience in emergency shelter environments.
- Experience working with vulnerable populations, including those with chronic health conditions, behavioral health issues, and housing instability.
- Experience in case management, housing navigation, social services, or healthcare, especially within Medi-Cal or CalAIM programs.
- Bilingual proficiency in Spanish and English is preferred.
- Ability to pass a comprehensive background check.
- Valid California driver’s license and a fully insurable driving record.
- Current Domestic Violence Counselor certification meeting California Evidence Code 1037.1 requirements or a willingness to obtain certification upon employment.
- Current Sexual Assault Counselor certification meeting California Evidence Code 1305.2 requirements or a willingness to obtain certification upon employment.
- Commitment to staying informed about best practices in Domestic Violence and Sexual Assault and resources available to shelter residents and to the community.
- Ability to foster community relationships on behalf of Casa de Esperanza.
- Strong interpersonal skills to build supportive and respectful relationships with staff and residents.
- Proven ability to work independently and collaboratively as an effective team member and leader.
- Must demonstrate reliability and consistency in attendance and performance.
