Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented Trans and Non-binary Supportive Housing Case Manager. The TNBH Case Manager will work in Cascade AIDS Project’s Housing & Supportive Services Department to provide services to a caseload of 15 individuals who identify as transgender and/or nonbinary regardless of HIV status which are identified through Coordinated Access, experiencing homelessness and may have co-occurring mental health and substance use issues.
This is a Full-Time (40 hours a week) role working at our Davis St. location in the Old Town area of Portland, OR. But travel throughout the 5 counties is required. The schedule for this role is a Hybrid (at least 3 days in office) Monday-Friday 8:30am-5:00pm
Starting pay is a yearly salary of $47,370.59. The TNBH Case Manager’s pay increases with tenure and the top of the pay scale is $55,170.59. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.
Who You Are
- One year experience relevant to the position or bachelor’s degree in human/social services field (social work, public or community health, psychology) or related field
- Prior experience providing case management (or similar) services
- Prior experience working with ethnic, racial, economic and sexually diverse populations and people experiencing homelessness, mental health, and/or substance use issues.
- Able to travel throughout the service area on a frequent basis.
- Valid Driver’s License and access to a reliable vehicle
- Available to work occasional evenings and weekends for mandatory CAP events
What You’ll Do
- Provide housing placement, supportive case management, and eviction prevention with and on behalf of clients who are homeless or at risk of becoming homeless using a supportive strengths-based model that promotes client self-determination and independence
- Work in collaboration with medical case managers, medical providers, or other providers involved in client care, to provide coordinated comprehensive care to Trans and Non-Binary participants with a focus on housing stability
- Establish professional relationships with landlords to create new housing resources and share the information with colleagues
- Work collaboratively with and communicate effectively with CAP staff, volunteers, community partners, and clients
- Attend trainings and educational opportunities to remain current on housing, HIV, case management, and other relevant professional issues
- Other duties as assigned
