What are the responsibilities and job description for the Behavioral Health Case Manager position at Cascade Comprehensive Care Inc?
This position is responsible for supporting behavioral health care provided by primary care providers in consultation with a team of behavioral health providers as appropriate and for coordinating referrals to clinically indicated services outside the primary care clinic such as social services, behavioral health specialty care, and substance abuse treatment by performing the following duties.
- Collects and assesses member information in order to develop a comprehensive, individualized care plan.
- Tracks member follow-up and clinical outcomes using appropriate electronic systems.
- Reviews referral and authorization requests for appropriateness of care within OHP guidelines.
- Monitors, coordinates and collaborates with community partners for Behavioral Health (BH) and Substance Use Disorders (SUD) services for members.
- Facilitates member engagement in their behavioral health care needs.
- Provides member education about common behavioral, substance abuse disorders and available treatment options.
- Documents in-person and telephone encounters in electronic systems to identify and track members for necessary follow-up.
- Facilitates care plan modifications for members who are not improving as expected in consultation with the PCP and the care team.
- Provides appropriate referrals for additional services when necessary.
- Coordinates Care Conferences with community partners to establish treatment plans for hard to manage members.
- Assists in appropriate generation and review of operating instructions and clinical guidelines for treatment.
- Maintains confidentiality and complies with HIPAA rules and regulations.
- Maintains punctual, regular and predictable attendance.
- Works collaboratively in a team environment with a spirit of cooperation.
- Displays excellent communication skills including presentation, persuasion, and negotiation skills required in working with members and coworkers and including the ability to communicate effectively and remain calm and courteous under pressure.
- Respectfully takes direction from manager.
EDUCATION and/or EXPERIENCE
Bachelor’s degree in Psychology or related field from four-year college or university and three to five years related experience and/or training; or a Registered Nurse with current state license with three to five years related experience and/or training.
Must have a clinical understanding of medical and behavioral health/SUD terminology.
COMPUTER SKILLS
Job requires specialized computer skills. Must be adept at using various applications including database, spreadsheet, report writing, project management, graphics, word processing, presentation creation/editing, communicate by e-mail and use scheduling software.
CERTIFICATES, LICENSES, REGISTRATIONS
LCSW or comparable behavioral health professional with extensive case management experience and/or credentials is preferred.
