Job Description
Position Title: Support Coordinator
Department: Targeted Case Management
Reports To: Support Coordination Manager FLSA: Full-time nonexempt
Summary: The Support Coordinator is responsible for coordinating and monitoring the quality of services for persons served, ensuring the delivery of and the appropriateness of services.
Essential Functions:
Manages a caseload of moderate size and complexity.
Coordinate the scheduling of ISP meetings with the persons served, guardians, families, providers and other interested parties.
Complete or acquire necessary evaluations/assessments to accompany plan. (i.e.,
MOCABI , UR checklist, ICF-MR, Health Inventory, Action Plans, Reviews DMH57, IPC etc.)
Complete quarterly reviews and monitor service quality.
Provide ongoing consultation with the team to develop/modify program plans and interventions.
Obtain informed consent when it is required, with signatures from the appropriate parties, and review necessity monthly.
Serve as a liaison to person served, parents, guardians, providers and other interested parties.
Serve as an advocate for individuals receiving services/supports.
Assist persons served in obtaining services from community resources. (i.e., vocational, educational, psychiatric, speech therapy, occupational therapy, assistive devices)
Monitors services/supports to ensure that the terms of the authorization are being fulfilled by the provider, to check on quality of services, and to review consumer progress.
Track TCM provided and to submit logs in a timely manner.
Work with providers to develop a cooperative network in order to better support persons served.
Secondary Functions:
Visits service agencies, attends meetings, and confers with other Support Coordinators to become informed concerning community resources for persons with developmental disabilities.
Serve on assigned committees.
Provide written follow-ups to assure services/supports are accessed.
Ensure compliance with HIPPA regulations in relation to documents, case notes, and files.
Serve as a resource to improve the delivery of non-DMH services/supports to people with disabilities.
Review quarterly financial statements for accuracy and ensure money is being spent appropriately and benefits are not in jeopardy.
Forward copies of quarterly financial statements to family, guardians.
Complete Medicaid reauthorization forms.
Attend all meetings and agency functions as required.
Perform all other duties as assigned by supervisor.
Request modifications for non-productive programs or enrolls the consumer in an appropriate program.
Qualifications:
Knowledge, Skills, and Abilities
Introductory knowledge of various developmental disabilities and corresponding providers and services available for persons served.
Ability to manage a caseload of persons served with developmental disabilities, to keep support plans current, and to maintain accurate records.
Ability to collect and analyze information to make decisions concerning a person’s served support plan.
Ability to develop a logical, feasible and practical support plan for clients with developmental disabilities.
Ability to evaluate the progress of persons served and the quality of their service programs.
Ability to communicate effectively.
Computer proficient, good communication skills, ability to get along with others, good organizational skills, negotiation skills.
Education: Bachelor's degree in Psychology, Special Education, Social Work or a related field with one or more years working with people with disabilities.
Certification: Bachelor’s degree in related field
Training: As required by job duties

Physical Requirements:
Regular attendance is essential. Ability to communicate effectively orally and in writing; works in office environment, but also frequently attends meetings outside of office environment.
Work Environment:
The work environment may be noisy with many different activities occurring simultaneously. This position involves direct interaction with a variety of people both in and out of the Casco offices.
Many people served by Casco have disabilities. Disabilities may include severe physical and behavioral disabilities. The employee in this position may experience verbally abusive behavior, sexually inappropriate behavior or physical aggression by consumers as a normal part of this job.
