What are the responsibilities and job description for the Case Manager position at Castle Senior Living?
ResponsibilitiesCoordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.Handle case assignments, draft service plans, and review case progress on an ongoing basis.Help clients achieve wellness and autonomy.Facilitate multiple care aspects (case coordination, information sharing, etc).Help residents make informed decisions by acting as their advocate regarding their clinical status and treatment options.Develop effective working relations and cooperate with the medical team throughout the entire case management process.Interact with patients to keep track of their progress and ensure satisfaction.Record case information, complete all necessary forms accurately, and produce statistical reports.Promote quality and cost-effective interventions and outcomes.Assess and address motivational and psychosocial issues.Adhere to professional standards as outlined by protocols, rules, and regulations.QualificationsAchieved a Bachelor's Degree in Human Services, Case Management, Social Work, or equivalent.Case Management experience in assisted living or equivalent.Experience working with the adult or geriatric population.Good communication, computer, and people skills.Able to establish working relationships with residents and families.Ability to work as part of a team.Legally authorized to work in the United States.Covid-19 Vaccine required.Ability to commute / relocate : Reliably commute or planning to relocate before starting work.Bachelor's (Required).BenefitsSalary : From $55,000-$70,000.Excellent salary & benefits.Paid time off.Beautiful residential setting and outdoor grounds.Ongoing continuing education and training events at the facility.401K options.Referral program.Retirement plan.#J-18808-Ljbffr
