Job Description: Case Management Coordinator
Requirements:

- Independent licensure with Supervisory Endorsement (LPCC-S, LISW-S, IMFT-S)
- 1-2 years leadership experience, preferred
- Experience working as a case manager, preferred
Responsibilities:
- Work and Clinical Supervision of Team of Case Managers
- Overseeing Case Manager job responsibilities (Crisis Intervention, Mental Health Support and Education, Referrals to Community Resources and Agencies, Home Visits, Professional Development and Training, Prevention and Education Groups)
- Monitoring High Risk Tracking Sheets
- Monitor documentation of Case Managers
- Monitor Referral and Active Student Spreadsheets
- Providing weekly supervision sessions with each Case Manager individually
- Documenting supervision sessions (Keeping supervision log and addressing issues below)
- Staffing cases
- Self Care/Wellness of Case Manager
- Legal/Ethical Issues
- Administrative Issues or School Building Issues/Communication with School Personnel
- Documentation/Outcomes/Communication Spreadsheets
- Documenting supervision sessions (Keeping supervision log and addressing issues below)
- Providing monthly team meetings with Case Managers
- Providing Catalyst updates, new community resources, etc
- Establishing meeting agendas and documenting meeting minutes
- Providing Catalyst updates, new community resources, etc
- Supporting and providing coaching for Case Managers between formal meetings
- Informal Check Ins and Availability for Crisis Consultation
- Training and education of Case Managers based on strengths and areas for growth
- Relationship building with District administrators (building admin, counselors, teachers)
- Training Case Managers about relationship building
- Organizational skills
- Communication Skills
- Knowledge of community resources, referral sources, etc
- Crisis Intervention Skills
- Relationship building with District administrators (building admin, counselors, teachers)
- Communication with District and School Building Administrators
- Quarterly meetings with each Case Manager school building admin
- Gathering and communicating outcome and satisfaction data to building, district administration, school building, and Catalyst Leadership
- Attending weekly supervision meetings with Chief Services Officer to review Case Management programming, supports needed and clinical consultation
- Managing and updating any changes needed to Case Management Job Responsibilities
