Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing comprehensive case management services. This position involves assessing client needs, developing personalized service plans, and connecting clients with appropriate resources to promote their well-being and self-sufficiency. The Case Manager will work collaboratively with various stakeholders to ensure clients receive the support they need.
Responsibilities (including but not limited to):
- Work with clients to optimize their autonomy and independence by having them lead, control, and exercise choice over their life goals, and the services and supports that assist their recovery and resilience.
- Ensure each client is being empowered and supported
- Serve as members of the recovery team and encourage the team to consider community resources
- Explain and teach clients about their rights and make sure they understand the options of using formal and informal grievance resolution processes.
- Provide direct assistance and support to clients.
- Check in with participants on scheduled and emergent bases to make sure that service plans are being followed and continue to be useful in meeting their life goals.
- Maintain a professional and cooperative relationship with all programs involved in providing services to the individual/family.
Duties (including but not limited to):
- Build rapport with individuals using a strength based, trauma informed, recovery oriented, and culturally sensitive approach.
- Create recovery plans with clients, family members, and identified team members focusing on strengths, goals, desired outcomes, and clients' priorities
- Complete Comprehensive Assessments
- Ensure the formation of a Coordinated Service Team for each participant, facilitate team meetings, and engage in follow up activities as necessary.
- Effectively document services provided
- Coordinate and facilitate mental health and/or AODA services with area schools, medical facilities, social service agencies and other professional agencies.
- Provide home visits, transportation to appointments, and accompany consumers to service appointments related to treatment when appropriate.
- Conduct recovery plan reviews and updates a minimum of every six months.
- Attend supervision, case management weekly meetings, and other meetings as requested.
- Participate in continuing educational opportunities, which promote personal and professional growth.
Qualifications:
- Minimum of a Bachelor's Degree in psychology, social work or related field required
- Minimum of 2 years experience working with individuals struggling with homelessness preferred
- Must have a valid driver's license, vehicle, and current personal auto insurance coverage and maintain all as a condition of employment
- Pass Criminal and Caregiver Background Checks per DHS 12
- Experience maintaining extensive client file documentation.
- Minimum of two years experience providing case management or service facilitation to people that have behavioral health challenges
- Excellent oral and written communication skills
- Excellent creative problem solving abilities and interpersonal skills
- Strong knowledge of Google Workspace, Microsoft Word, and Microsoft Excel
- Strong working knowledge of mental health issues and substance use disorders.
- Ability to work and communicate frankly, respectfully, and effectively with children, youth, adults, families, and all team members
Job Type: Full-time
Pay: From $52,000.00 per year
