What are the responsibilities and job description for the Substance Abuse Counselor position at Catalyst Health Solutions?
NOTE: Salary Range not Set and Negotiable Depending on Experience
To provide case management, counseling, and treatment planning with patients who are engaged in medication assisted treatment for opioid use disorder as well as poly substance abuse.
See patients/clients in office individually or in group setting to assist in facilitating change, creating relapse prevention plans, and connecting patient/client to local resources to meet basic needs.
Develop client monthly treatment plans based on research, clinical experience, and client input as well as review and evaluate clients' progress in relation to measurable goals described in treatment and care plans.
Participate in multidisciplinary team meetings as well as supervision to facilitate growth.
Comply with all HIPAA regulations, mandated reporting duties, as well as documenting progress appropriately.
Coordinate care with courts, probation officers, community services and other agencies involved with patient.
Flexibility regarding transportation and shift coverage at various locations within the Catalyst Health Solutions network.
Ability to work in fast paced, high volume atmosphere.
Knowledge of the ARTS program preferred.
Flexible team player with a positive attitude towards work/home balance.
Required Credentials : LMSW, CSAC, LSAT, LPC, QMHP (within the state of VA)
Job Type: Full-time
Pay: $50,000.00 - $60,000.00 per year
